The Public Health Nursing Standard 13 describes collaboration as an essential component of the nursing practice which involves collaborating with healthcare consumers, families, and others healthcare professionals and disciplines. The goal of collaboration is to partners with others to share knowledge and situations of individuals being cared for; to create a documented plan focused on outcomes; and to resolve conflicts within the context of patient care to effect change and produce positive outcomes. Achieving exceptional collaboration among team members requires adherence to codes of conduct that govern behavior among peers and colleagues to create a cooperative, respectful, and trusting work environment (American Nurses Association, 2013).
In my community hospital practicum setting, the main methods of collaboration that I have observed between internal and external stakeholders is face-to-face assessments between patients, nurses, physicians and case managers; interdisciplinary rounding by nurses, physicians and management; secure text message from nurses to attending, on call and consulting providers; telephone calls; clinical notes documented by interdisciplinary healthcare providers; written after visit summary of patient care that was provided. The practicum site is equipped with the equipment to serve the needs of workers and the patients, such as individual handset phones for nurses, attending and on call physicians; fax machines, computers with secure text software, and e-mail; internal secure employee portal website; patient portals and electronic health record; interpreter phones and video interpreter and conferencing equipment. In many cases patient documents need to be faxed between the hospital and outside community providers, such as skilled nursing facilities. Having this technology available speeds up the process.
