Historically there are many ethical dilemmas in public health that have been for decades that is affecting the ethical dilemma. The perception that vaccine causes autism, which is a neurological disorder with unknow etiology, this has affected mothers who want to make safe choices for their children withhold the vaccinations (Roberts & Harford, 2002).

African American community and rural area population who avoid vaccine for years has stretched to the current pandemic. The federal data of covid-19 vaccination by race shows that 60% white have been vaccinated with only 11% Black, 17% Hispanic, yet the Black community continue ti be most likely to be affected or diet from COVID-19 when compared to other races (Ndugga, N. et al., 2021), and many refusing the vaccine that have saved many lives.
