In this assignment we are going to begin to apply the concepts we have been learning for quality improvement. You will identify an opportunity for improvement, and apply the Model for Improvement to address it. In addition, you will think about the HIM implications of your plan.
Step 1:
Think about the last time you had an encounter with the healthcare system: the last time you saw your doctor or any doctor; the last time you took your friend to the doctor; the last time you visited the ED, had a test, visited a friend in the hospital, and so on. Consider, in that experience, what could have gone better. Be sure to select a situation where the issue is due to system design (eg wait time was too long), as opposed to a personal issue (eg someone was rude). Discuss the encounter.
Step 2:
Now, use that encounter as a basic tenet of quality improvement (QI). Set your “aim” for QI. Remember to make it specific and time limited. An example would be: “This project will decrease the amount of time people wait to be seen by 50% in 6 months.” (No, you may not use long wait time as your encounter!). 🙂
Step 3:
Next, design a family of measures for your QI project. Make sure to include at least one outcome measure, one process measure, and either one structure, patient experience or balancing measure for a total of three measures (see Spath, pages 44-45, and QI 102). Discuss how your HIM system might help you collect this data (refer to PPT slides for help).
Step 4:
Read the experiences your classmates have posted. Respond to at least two peers by analyzing the strengths and potential weaknesses of their project aim and measurement strategy (remember to provide constructive comments in a professional manner). Demonstrate your critical thinking skills here! 🙂
