For this assignment, you will assume that you are employed by the local Health Department as a public health professional. You have been approached by a funder, called the Champion for a Healthy City Foundation, to submit a proposal for a grant.

The Foundation has shared that grants will be awarded in the amount of up to $100,000 for a 12-month program.

Your Program Proposal will be broken down into four (4) written parts:

Program Proposal Part 1 will be a 2-3 page rationale that includes relevant and specific data and background on the health issue and community chosen (as identified in the CHA and CHIP), a health behavior theory or model and at least one evidence-based strategy.

The Program Proposal Part 2 will have a goal statement, 2 SMART process objectives, 2 SMART impact objectives, a Gantt chart (or timeline) and a completed logic model.

The Program Proposal Part 3 will include the 4-5 page program narrative.

The Program Proposal Part 4 will include a detailed budget (a template will be provided), a 1-2 page evaluation plan and a description of at least 2 deliverables (marketing tool, assessment tool, lesson plan, developed curriculum, etc.).

The Program Proposal Part 2 will have a goal statement, 2 SMART process objectives, 2 SMART impact objectives, a Gantt chart (or timeline) and a completed logic model. This is worth 20 points.
