Discussion 1: Jordan
A research problem is a gap in nursing knowledge related directly to nursing practice (Gray & Grove, 2020). I interviewed my units nursing educator regarding our central line dressing change policy. Our current policy states that central line dressings should be changed 24 hours post-insertion and then weekly. I have always challenged the need to change a dressing 24 hours after insertion as I feel it increases the risk for infection. The policy does not stipulate to change the dressing based on whether it is clean or dirty, simply that it should be changed within the 24-hour timeframe. I have asked multiple people within my department and others regarding the evidence for this provision, and none have referenced it as best practice.
My greatest takeaway from the interview was the need to challenge the status quo and the importance of solid evidence to support an argument. As a small rural hospital, we all have to wear many hats, and as a result, things such as policy can be overlooked. It is the responsibility of everyone to challenge the status quo and present new evidence for policy change when areas of improvement are recognized.
We also discussed the importance of stakeholders when attempting to implement policy change. Stakeholders are an integral part of disseminating and implementing change (Artime et al., 2021). Stakeholders add credibility to the proposed change and help to gain traction among the decision-makers.
The interview reinforced the purpose of evidence-based practice. It is easy to see opportunities for change but difficult to implement change. Having evidence that supports the need for change strengthens an argument. This concept will help guide me as I work through my evidence-based project of changing our policy on central line dressing changes.
Discussion 2: Jennifer
I interviewed MK who has a Masters in Nursing Education. The interview consisted of me asking what recommendations she has for me regarding gaps in research, what journal articles she recommends for me to read, a relevant organization for me to join or participate in, and any tips for being a clinician researcher in nursing research.
Recommendations that MK had for me was to go over the policy and procedures regarding our department and procedures that we conduct as well as all education that we receive in the area to make sure I do not miss standards of practice. MK also recommended that I do a lot of brainstorming and ask questions about the topic that I chose to make sure I am addressing the right question.
MK suggested that I read articles related to nursing regarding safe patient handling. She also recommended articles that related to perioperative procedures. She stressed the importance of knowing about the procedures and outcomes from the procedures for best practice. The evidence-based knowledge behind pre and post procedures will help validate where to prep and recover patients safely.
MK suggested that the American Nursing Association is an organization that might be able to help me with support of nurse patient ratios and safe nursing staffing. She also suggested being a part of unit-based teams to help with polling of the department. MK also suggested to get involved with the educators and see how they can help with research.
MK said not to get frustrated and that it takes time. She also said not everyone will be involved with the project when it comes to helping with questionnaires and surveys and that can get frustrating.
The interview helped me understand that there is a lot behind nursing research. MK helped me to understand that this is something that you must get through to graduate. She and I have had many frustrations at work and the way things are done so I also realized that if I want to change something regarding nursing evidence-based practice is the best way to go about it as well as knowing nursing research. The ultimate goal if nursing is to provide evidence-based care that promotes quality outcomes for patients, families, healthcare providers, and the healthcare system (Gray, J & Grove, S., 2021, P. 7).
MK and I have worked together for a few months now and she understands my passion for patient care and patient safety. I see daily when I am at work the struggle to get some people involved in changing a process or voicing a concern. The response I hear a lot is because that is the way we have always done it and that frustrates me. I hear quite often it can always be worse. The interview just helped me to be innovated, know to have patience and not give up and to read and ask a lot of questions. Innovations and changes in the field of health have revealed the importance of using evidence-based practices in treatment and care practices (Erol et al., 2022, p. 33).
