A step for addressing a concern of discharge planning is for nurses to participate in interprofessional care teams (ICT). The evidence-based practice supports a reduction in rehospitalization risks and avoids communication concerns during hospitalization. The nurses involvement impacts the quality of care for patients. Nurses participation in the interprofessional discharge planning process is needed to identify barriers, collaborate, and communicate promptly to avoid discharge delays. Patients readmitted to the hospital are linked to poor communication and discharge planning. A poor patient discharge has barriers in the discharge planning process: system insufficiencies, reconciling medications, staff shortage, discharge timing, and lack of communication between members of the interprofessional teams. Healthcare will need to develop and train multidisciplinary health teams and non-medical staff and use technology and evidence-based practice (EBP) to provide positive outcomes for our most susceptible population. ICT collaboration and communication cost-saving is patient satisfaction, employee satisfaction, proper medication reconciliation, and appropriate resources allocated (Prusaczyk et al., 2019).
A recent study showed the relationship of using EBP based on Project RED (re-engineered discharge) of 79 hospitals to have improved outcomes for discharges from the hospital. The use of survey scores proved a higher level of quality improvement with EBP to discharge patients. EBP is underutilized in many hospitals, and the groundwork for incorporating more has taken decades to implement. Determining the size and location characteristics of the organization helps to tailor new practices and strategies to start EBP utilizing the Project RED model (Innis et al., 2021).
